Washington Prevention Provider Meeting 2016

Sunday, November 6, 2016 9:30 am-4:30 pm – The Washington Prevention Provider Meeting 2016 is open to everyone.  This meeting is a requirement for Community Prevention and Wellness Initiative (CPWI) ESD staff, County Prevention staff (if applicable) / Community Coalition Coordinator or their designee. Registration for this event is separate from the Prevention Summit Conference.  To register for this meeting only, please click here.

Adult Program Registration

Non-chaperone adult registration begins on August 1 and will be able to register up until the day of the conference. The early rate ends on September 13 for registration costs.

Team Adult Advisor/Chaperone and Youth Team Registration

Teams will be able to register online from August 1 – October 31 only. First-come, first-served. All teams must register and arrange for payment (Purchase Order, credit card, check) within this time period to attend the Prevention Summit.

For youth teams that have missed the online registration period, but would still like to attend the Prevention Summit, please contact us by emailing contactus@preventionsummit.org. We will provide you with paper registration forms which you may complete and bring to the Yakima Convention Center to complete registration onsite.

All youth teams must also bring completed and signed youth permission forms to the Summit!!!

Payments

Payments can be made by credit card, check, or purchase order using the online registration system. For checks and purchase orders, please make them payable to ‘Board of Regents’ and reference the participant(s). Please mail payment to the address below.

Cancellation, Substitution, and Refund Policy

Cancellations
If you are unable to attend the conference, please send a written cancellation notice to CASAT no later than Thursday, October 6, 2016 by 5:00pm PDT* for a full refund. If your cancellation notice is received after the deadline a refund will not be possible. Please send a request in writing to contactus@preventionsummit.org with “Cancellation” in the Subject line. Note: An unpaid “no show” still owes the conference hosts for the cost of registration.  That is, if you do not cancel your registration by the cancellation deadline and do not attend the conference, you still owe the conference hosts as costs have already been incurred for the head count.

Substitutions
Substitutions can be made (until Thursday, October 6, 2016) by following instructions listed in your confirmation email or by sending a written notice, with “Substitution” in the subject line to contactus@preventionsummit.org . Please include the name(s) and registration information of replacement(s), if applicable. There will be NO ADULT SUBSTITUTIONS for a registration after this date. Youth may be substituted, as we are aware of the complications of confirming youth participation with school and family variables. There are no onsite “substitutions” allowed.

Refunds
To receive a full refund for a cancelled registration, your cancellation must be received prior to the cancellation deadline (Thursday, October 6, 2016). It is the registrant’s responsibility to follow up with CASAT within two (2) months after the conference in the event a refund is not received. If a registrant does not follow up on a refund and it is not received, no refund will be possible following this timeframe.
